Checklist for Newly Admitted Students

This page is your checklist to help you make sure that you have completed all of the forms and requirements and are ready to start your first semester here at UMC. You must also be sure that you have all of the required documents with you when you attend Orientation at UMC.

1. Fill out the Medical History Form and mail it to Student Health Services on campus (address is on the form). Upon your arrival at UMC you will be visited by a nurse to assure that your medical records are up to date; you will also need to have $15.00 with you to pay for immunizations.
2. Non citizens of the United States must have had a Mantoux Test done, a test for tuberculosis. If you have already done so you need to bring the documentation with you. If there are questions regarding your test, you may be required to retest. Be sure to tell the nurse if you have recently been immunized for tuberculosis because retesting may cause a reaction to the Mantoux test.
3. Nonimmigrant status documents (I-20's, I-94's, Visa Passport, and any other documentation's) must be approved, and you need to have them with you upon arrival.
4. If you are a sponsored student you need to bring all documents related to your sponsorship.
