Waiver of Document of Privacy

I agree that by proceeding to and subsequently submitting an electronic application for registration with Counseling and Career Services. I am permitting the University of Minnesota, Crookston Counseling and Career Services to make my resume, transcript, electronic file and any other documents downloaded to my Counseling and Career Services account available to employers associated with the Counseling and Career Services. In addition, I agree that my electronic and paper file, maintained by the Counseling and Career Services may be reviewed by faculty and staff of the college in the process of monitoring my academic progress.
